Instructions for Completion and Application

Please have a check for $50 made out to Mod Realty, Inc. for each applicant over the age of 18 who will be residing in the apartment.  (This is a credit check fee).

 

Have a money order or a cashier's check for the 1st month rent made out to Mod Realty, Inc.

 

We will also need your proof of employment. Please include it with your application. We accept two types of proof of income:

• Your last 2 pay-stub checks

• A signed letter from human resources/your supervisor stating how long you have been working at your present job, your salary (any upcoming bonuses) and your position

 

Deliver all the above documents to: Mod Realty, Inc. 1227 N Paulina, Chicago, IL 60622.

 

Once we have reviewed your documents and upon approval, we will contact you to schedule a time for you to sign the lease.

 

It is at the lease signing when the move-in fee is due, $300 per person for whomever is signing on the lease.
